ESC LHIN Smoking Cessation Care Path Planning

Working groups have been initiated including all three Public Health Units to review plans around the development of smoking cessation care paths and to build upon the work that has been taking place in these communities. As noted in the IHSP 3, under the Strategic Direction of Improving Outcomes in Chronic Disease Management, a success measure of reducing the daily or occasional smoking rate within three years from 22.2 per cent to 18.9 per cent was set. 

The first working group will be established in Chatham-Kent, in collaboration with the CK Counts project, in July. Meetings are scheduled with the Windsor-Essex County Health Unit and the County of Lambton, Community Health Services Department to discuss future plans in these areas.
