Assess and Restore Program a Success

The Assess and Restore Program was first launched at Leamington District Memorial Hospital as a pilot project on August 1, 2010, yielding very positive results. As of October 21, 2010, patients of the Assess and Restore program were discharged to:

  • Home without CCAC 13%
  • Home with CCAC 57%
  • Rest Home with CCAC 15%
  • Acute Care 6%
  • ALC Designation 9%

Making it Work for Hôtel-Dieu Grace Hospital

It is expected that the new unit at Hôtel-Dieu Grace Hospital will yield similar results and positively impact the high ALC rates and the systemic effects they cause.

Operating a new 16-bed unit, it is expected that Hôtel-Dieu Grace Hospital will acquire new staffing. Specifically, a multidisciplinary care team will provide restorative care for seniors admitted to the unit, including Nursing (RN and RPN), Pharmacist, Occupational and Physiotherapist, Social Worker, and full time CCAC Case Manager.
