100 per cent connected
All 67 acute care hospital sites providing data as part of the integrated electronic health record
As of July 16, 2015, 100 per cent of the 67 acute care hospital sites in south west Ontario (SWO) now provide data as part of the integrated electronic health record (EHR) for Ontarians.
Data from the 67 acute care hospital sites in south west Ontario is now accessible in the cSWO Regional Clinical Viewer, ClinicalConnectTM, following the successful integration of the remaining six hospital sites in the Erie St. Clair Local Health Integration Network (ESC LHIN). This milestone represents significant robustness of the integrated EHR, as well as achievement of the cSWO Program’s acute hospital integration target.
The following hospital sites’ data, including diagnostic images each facility contributes to the Southwestern Ontario Diagnostic Imaging Network (SWODIN), is available in ClinicalConnect:
- Chatham-Kent Health Alliance
- Chatham Campus
- Sydenham Campus
- Hôtel-Dieu Grace Healthcare
- Leamington District Memorial Hospital
- Windsor Regional Hospital
- Metropolitan Campus
- Ouellette Campus
In addition, data from the regional cancer care programs and the community care access centres (CCACs) in each of the four SWO local health integration networks (LHINs) is available through ClinicalConnect. As well, lab requests and results from Ontario laboratories information system (OLIS) and diagnostic images from 53 hospital sites through SWODIN are now integrated in ClinicalConnect. Data from the nursing assessment repository (Health Outcomes for Better Information &Care (HOBIC) is also available.
“I am very excited about Chatham-Kent Health Alliance coming online with ClinicalConnect. As an urologist in Sarnia, we receive a lot of patients from the Chatham area and it will be great to be able to access their information. ClinicalConnect is easy to use,” says Dr. Paul Martin, Urologist, Bluewater Health.

The connecting South West Ontario (cSWO) Program is a regional ehealth program, funded by eHealth Ontario.
