Patients with Heart Disease Now Have Access to New Cardiac Care Services in Sarnia/Lambton
Residents of Sarnia/Lambton now have access to new cardiac care services through the creation of the Heart Failure Clinic at the North Lambton Community Health Centre. Thanks to an investment of $122,700 by the Erie St. Clair LHIN, people who have experienced heart failure can now access services dedicated to improving the management of their cardiac care.
Through this investment, the North Lambton Community Health Centre has appointed a Nurse Practitioner to provide efficient, community-based, access to specialized care in a standard multidisciplinary heart failure clinic model. Primary Care Providers and Specialists will work collaboratively with the Nurse Practitioner to share in important aspects of care for patients.
Over the last two decades, heart failure has been recognized as a significant challenge in both hospitals and in the community. According to the Cardiac Care Network, appropriate management and support of heart failure in the community is critical to ensuring optimal quality of life for patients and their caregivers, while minimizing the burden on Ontario’s health care system.
The North Lambton Community Health Centre has committed to work with community partners to establish an inter-professional collaborative model of patient care between the Nurse Practitioners, Cardiologists, Internal Medicine Specialists, Primary Care Providers, and existing Allied Health teams to deploy a “hub and spoke” organization of team care. Services will be based on individual patient needs, incorporate the latest best practices, and give patients the tools to self-manage their disease.
The development of the new Heart Failure Clinic is possible thanks to the partnership between the North Lambton Community Health Centre and the Mackenzie Clinic Vascular Protection and Cardiac Rehabilitation Program. Through this alignment the clinic is posed to aid many people in the community, and its success will form the springboard for future development.
