CCAC to Support More Complex Care in Home
The ESC LHIN is pleased to provide funding in the amount of $225,000 (one time) to the CCAC for the Intensive Hospital to Home Initiative (IHH). IHH supports patients with high acuity care needs to enable them to:
- Stay in their homes as long as possible
- Transition from hospital to home to make decisions about where their care needs are best met in the future
- Support increased volumes of high needs patients from community agencies, including primary care
The IHH supports through CCAC will also assist with improving patient flow through the acute and post-acute care hospital system, by being able to care for more complex patients at home and reducing the need for increased hospital stays.
This investment delivers patient care in response to the distinct local needs in the Erie St. Clair LHIN. The ESC LHIN harnesses local expertise to plan, coordinate, and finance health care through provincial funding and in partnership with local health service providers. This investment supports the commitments made in Patients First: A Roadmap to Strengthen Home and Community Care, Ontario’s plan to improve and expand home and community care, and is part of Ontario’s increased funding for that sector of more than $750 million over three years.
