Erie St. Clair Local Health Integration Network Seeking Patient and Family Advisory Committee Members

The Erie St. Clair Local Health Integration Network (ESC LHIN) is looking for members to sit on its Patient and Family Advisory Committee.
Since its inception, the ESC LHIN has recognized the value of listening to the voices of patients and their families/caregivers. We believe that when people are actively involved in managing their own health and wellness, as well as creating their own health care and community solutions, they will live healthier lives in healthier communities.

The ESC LHIN’s Patient and Family Advisory Committee will provide the patient perspective to LHIN programs, services, and initiatives – including sub-region tables – through sharing stories, advice, and experiences that reflect the reality of all people living in our region.Patient and Family Advisory Committee membership is extended to all ESC LHIN community residents and will reflect the diversity of our communities. 

Interested members need to have health care experiences within the past two years. This opportunity will require a commitment to attend a minimum of six meetings a year in Chatham in person or via teleconference or video conference.

Applications must be received by Friday, December 7, 2018.
